We want to hear about your overall experience with Wildwood Counseling Services - including what is going well, what has been helpful, and where we may be able to improve.
Our Client Feedback Survey is a place to share positive feedback, suggestions, ideas, or general comments about your experience with your provider, medication management, scheduling, communication, office environment, billing, telehealth, or any other part of your experience with Wildwood.
You may provide your name or choose not to identify yourself. Providing contact information is completely optional unless you would like someone from Wildwood to follow up with you.
If you have a specific concern about something that happened and would like Wildwood to review, address, or respond to it, please use the Client Concern & Grievance Form instead. If you are unsure which form fits best, that is completely okay — you can use whichever feels most comfortable, and we will make sure your feedback is directed appropriately.
What Happens Next?
Client feedback is reviewed by Cassandra Reineck, owner of Wildwood Counseling Services, and may be used to help us identify areas where we are doing well, recognize providers or staff, and make improvements to our services and practice.
If you request follow-up and provide your contact information, someone from Wildwood may reach out to you regarding your feedback.
If something shared through the Client Feedback Survey raises a concern related to safety, professional boundaries, privacy, quality of care, or another issue that may require further review, Wildwood may follow up or address the information through our grievance, quality-assurance, or required reporting processes as appropriate.
Share a Concern
If you have a concern about your care or experience with Wildwood Counseling Services, we want to make it easy for you to share it. Concerns may involve your provider, professional boundaries, communication, medication management, billing, privacy, scheduling, office interactions, or any other aspect of your experience with our practice.
You are welcome to raise a concern directly with your provider, contact Wildwood administration, or complete the confidential Client Concern & Grievance Form below.
You do not have to know whether something was “wrong” or a violation in order to tell us that an interaction made you uncomfortable or concerned.
You may provide your name or choose not to identify yourself. Providing contact information allows us to follow up with you, but it is not required.
Wildwood will not retaliate against you or treat you adversely because you raise a concern or grievance.
What Happens Next?
Once you have completed the form, Cassandra Reineck, the owner of Wildwood, will review the information provided and determine appropriate next steps. If you request follow-up and provide contact information, we will make reasonable efforts to acknowledge your concern within five business days.
Depending on the nature of the concern, additional review or reporting to an appropriate licensing, regulatory, protective, or governmental agency may be required.
You Can Also Report a Concern Outside of Wildwood
You are not required to complete Wildwood’s internal grievance process before contacting an outside licensing or regulatory agency.
If you are unsure which agency regulates your provider, Wildwood can help you identify the appropriate licensing board.
